Az előző részben bejutottunk Smoke titkos bázisára és elpusztítottuk. De Smoke nem hagyta annyiban, visszatért a halálból egy titkos gép segítségével és bosszút esküdött CJ ellen. Az évek során Smoke rendgeteg ellátmányt és járművet lopott össze magánhadseregével. Itt az hogy egy komoly bosszút álljon.
